Resumen
Introducción. La actividad minera representa una fuente importante de exposición ocupacional y ambiental a contaminantes con potencial hematotóxico, incluyendo metales pesados, compuestos orgánicos volátiles y polvos minerales. Sin embargo, las alteraciones hematológicas asociadas a la minería han recibido menor atención en comparación con otros efectos sistémicos. El objetivo de esta revisión fue sintetizar la evidencia disponible sobre la asociación entre exposición a la actividad minera y enfermedades hematológicas, considerando contaminantes implicados, mecanismos fisiopatológicos y principales alteraciones hematológicas descritas. Material y métodos. Se realizó una revisión narrativa de la literatura mediante búsqueda en Pub- Med/MEDLINE, Scopus y Web of Science hasta el 30 de abril de 2026. Se incluyeron estudios observacionales, experimentales, revisiones sistemáticas y metaanálisis publicados en inglés y español relacionados con minería, exposición a contaminantes y hematotoxicidad. Resultados. La evidencia muestra que la exposición minera se asocia con alteraciones en múltiples líneas celulares sanguíneas. En la serie eritrocitaria, se describen disminuciones de hemoglobina, hematocrito y eritrocitos, compatibles con anemia. También se reportan alteraciones leucocitarias, incluyendo linfopenia relativa, neutrofilia y cambios en la relación neutrófilos/linfocitos, así como modificaciones en parámetros plaquetarios y hemorreológicos. Los principales mecanismos implicados incluyen estrés oxidativo, inflamación sistémica crónica, inmunotoxicidad, interferencia con la síntesis del hemo y daño genético sobre células hematopoyéticas. La exposición prolongada a contaminantes como plomo, mercurio, arsénico y benceno se relaciona además con mayor riesgo de citopenias persistentes y neoplasias hematológicas. Discusión. La minería constituye un problema de salud pública por su potencial impacto sobre el sistema hematopoyético. Se requieren estrategias integradas de vigilancia ocupacional y ambiental, así como estudios longitudinales que permitan comprender mejor los efectos acumulativos y las exposiciones combinadas.
