Abstract

La automatización del laboratorio hematológico ha evolucionado considerablemente durante las últimas décadas, permitiendo incorporar parámetros celulares avanzados que aportan información diagnóstica adicional más allá del hemograma convencional. Dentro de estos biomarcadores, la hemoglobina reticulocitaria, reportada como CHr (Content Hemoglobin Reticulocyte), RET-He (Reticulocyte Hemoglobin Equivalent), MCHr (Mean Cellular Hemoglobin of Reticulocytes) o RHE (Reticulocyte Hemoglobin Expression) según la plataforma analítica empleada, se ha consolidado como un indicador útil para evaluar la disponibilidad funcional de hierro durante la eritropoyesis reciente. A diferencia de los marcadores bioquímicos tradicionales, este parámetro refleja la cantidad efectiva de hierro incorporada a la síntesis de hemoglobina en médula ósea durante los días previos al análisis. Su utilidad clínica incluye la detección precoz de ferropenia, la identificación de deficiencia funcional de hierro y el monitoreo temprano de respuesta terapéutica. En esta ficha técnica se revisan sus fundamentos biológicos y analíticos, las principales variables preanalíticas, diferencias metodológicas entre plataformas disponibles, valores de referencia y aplicaciones prácticas.
