Abstract

Enteropathy-associated T-cell lymphoma is a rare and aggressive type of peripheral T-cell lymphoma, frequently associated with celiac disease. Refractory celiac disease (RCD), the main cause of lack of response to the diet may be developed after the inadvertent and prolonged ingestion of gluten. Two types of RCD are recognized: type I and type II, while type II is currently considered a pre-lymphomatous state. The persistence of an inflammatory stimulus posed by the continuous intake of gluten in celiac patients determine a disturbance of intestinal lymphocyte homeostasis, raising the possibility to evolve to a lymphoma. The target of malignant transformation appears to be an intraepithelial lymphocyte developing increased proliferation potential and apoptosis escape. A 62-year-old male case is presented as illustration of the possible relationship between these two entities as the mechanisms intertwined between.
