Resumen

El dímero D es un marcador de la generación de trombina y plasmina. Hace un tiempo que se ha demostrado la utilidad de la medición de dímero D como valor predictivo negativo en la exclusión de la enfermedad tromboembólica en pacientes con pretest clínico bajo o moderado. Sin embargo, la aparición de nuevas metodologías, que expresan los resultados en diferentes unidades y que tienen distinto grado de aprobación para su utilización en el algoritmo de exclusión de la enfermedad tromboembólica, ha generado cierto grado de confusión. El objetivo de esta revisión es recopilar y difundir las recomendaciones de las distintas guías internacionales sobre la utilización del dímero D en el diagnóstico de la enfermedad tromboembólica venosa. Se discuten los diferentes grados de aprobación de la Food and Drug Administration para los métodos de laboratorio (ayuda en el diagnóstico vs exclusión), las unidades en que se deben expresar los resultados, qué información debe figurar cuando se informa un resultado de dímero D, cómo establecer los puntos de corte de los distintos métodos y en qué pacientes puede utilizarse el ensayo de dímero D para excluir enfermedad tromboembólica.
